Bolivar Trask

Bolivar Trask developed the Sentinel program for the DEO. He went beyond the bounds of the design parameters and built a self-improving synthetic intelligence that was a supercontroller for the Sentinels, called Sentinel Alpha. The AI went rogue, killing thousands of metahumans. Trask was blamed for the deaths, and was set to fae extensive jail time when he was discovered dead in his prison cell in 2017.
